Original abstract

Hintergrund: Morbus Parkinson ist eine degenerative neurologische Erkrankung und betrifft in Österreich rund 20 000 Personen. Durch den fortschreitenden Untergang von dopaminergen Neuronen im Mittelhirn kommt es sowohl zu motorischen als auch nicht-motorischen Symptomausprägungen. Die Erkrankung äußert sich primär durch die Kardinalsymptome Brady-/Hypokinese, Rigor und Tremor. Im fortgeschrittenen Stadium kommt es zusätzlich zur posturalen Instabilität. Neben der pharmakologischen Therapie, sowie der physiotherapeutischen Intervention wurde in den vergangenen Jahren kontaktloses Boxen als ergänzende Therapiemaßnahme für Parkinson-Patient:innen untersucht und stellt einen wichtigen Eckpfeiler in der Behandlung dar. Fragestellung: Ziel dieser Arbeit ist es anhand von Literaturrecherchen den Effekt des therapeutischen Boxens als Therapieintervention zur Verbesserung der posturalen Stabilität bei Patient:innen mit Morbus Parkinson zu ermitteln. Dabei soll die Wirkung dieser Intervention anhand von sieben Hauptstudien evaluiert werden. Ergebnisse: Die Resultate der analysierten Literatur deuten darauf hin, dass therapeutisches Boxen zu einer Verbesserung der untersuchten Messparameter führte. In keiner der Studien wurde eine Kontrollgruppe verwendet. Stattdessen konzentrierten sich die Studien ausschließlich auf das therapeutische Boxen und dessen Auswirkungen auf die posturale Stabilität. Das Gleichgewicht wurde mittels verschiedener Assessments bewertet, wobei der Timed Up and Go Test, der Sit to Stand Test und die Fullerton Advanced Balance Skala am häufigsten verwendet wurden. Aus allen Studien konnten signifikante Verbesserungen entnommen werden, vor allem aus jeden, die eine längere Interventionsdauer aufweisen. Dies legt nahe, dass regelmäßiges, langfristiges Training von großer Bedeutung für die Wirksamkeit für Morbus Parkinson-Patient:innen ist. Die Ergebnisse der Studien zeigen zudem, dass es zu einer unmittelbaren Verbesserung der Ergebnisse nach Therapiebeginn gekommen ist. Conclusio: Aufgrund der aktuellen Studienlage kann man davon ausgehen, dass dieser therapeutische Ansatz bei Morbus Parkinson im Hoehn und Yahr Stadium eins bis vier empfohlen werden kann. Dennoch ist in den Studien der genaue Ablauf, wie die Intervention an die individuellen Bedürfnisse der Teilnehmer:innen mit mittel-schweren bis schweren Schweregrad angepasst wird, nicht eindeutig nachvollzieh-bar. Zudem wären in Zukunft weitere Untersuchungen zielführend, in welchen einerseits präzise Angaben zur Häufigkeit und Intensität des Boxtrainings und andererseits dem Pausenmanagement und der Belastbarkeit genannt werden. Background: Parkinson’s disease is a degenerative neurological disorder affecting approximately 20.000 people in Austria. The progressive degeneration of dopaminergic neurons in the midbrain leads to both motor and non-motor symptoms. The disease is primarily characterized by the cardinal symptoms or brady-/hypokinesia, rigidity and tremor. In advanced stages, postural instability may also occur. In addition to pharmacological and physiotherapeutic intervention, boxing therapy has been studied as an adjunctive therapy for Parkinson’s patients and represents an important pillar in treatment. Objectives: The aim of this study is to determine the effect of therapeutic boxing as a therapy intervention to improve postural stability in patients with Parkinson’s disease through literature research. Results: The results of the analyzed literature suggest that therapeutic boxing led to an im-provement in the measured parameters. In none of the studies a control group was used, i.e., the results of the effectiveness of therapeutic boxing were not compared to those of a group that had not undergone the training. Instead, the studies focused exclusively on therapeutic boxing and its effects on postural stability. Balance was assessed by using various assessments, with the Timed Up and Go test, the Sit to Stand test, and the Fullerton Advanced Balance scale being the most used. All studies were able to show significant results in terms of improvement, especially studies of longer intervention duration. This suggests that regular, longterm training is of great importance regarding the effectiveness for patients with Parkinson’s disease. The results of the studies also show that an improvement in outcome could already be detected immediately after the start of therapy. Conclusion: Based on current studies, it can be assumed that this therapeutic approach can be recommended for Parkinson’s disease in Hoehn und Yahr stages one to four. However, the exact process of how the intervention is tailored to the individual needs of participants with moderate to severe stages was not completely transparent in the studies. Additionally, in the future, further investigations would be beneficial, which provide precise information on the frequency, and intensity of boxing therapy as well as the management of breaks and tolerance levels.
