La stimulation cérébrale profonde (DBS) est une intervention thérapeutique utilisée pour atténuer les symptômes moteurs de la maladie de Parkinson, mais elle reste une procédure complexe et délicate. Cette thèse a pour objectif de développer une méthode prédictive robuste permettant d'identifier les zones optimales de stimulation cérébrale profonde tout en minimisant les risques d'effets secondaires pour les patients. Nous avons conçu un flux de travail intégrant une parcellisation des structures cérébrales profondes en sous-régions cohérentes. Ces sous-régions sont ensuite utilisées pour prédire l'effet de la stimulation cérébrale profonde à un point donné, à l'aide d'algorithmes d'apprentissage automatique. En analysant les prédictions du modèle en fonction de la position des électrodes, une région « optimale » a été mise en évidence. Cette zone se distingue par une amélioration clinique significative, tout en limitant le risque d'effets secondaires, aussi bien à l'échelle individuelle des patients qu'à celle d'un atlas cérébral.Cette méthode prédictive ouvre la voie à une personnalisation des traitements par stimulation cérébrale profonde, en optimisant l'efficacité thérapeutique et en réduisant les risques pour les patients. Deep brain stimulation (DBS) is a therapeutic intervention used to alleviate motor symptoms of Parkinson's disease, but it remains a complex and delicate procedure. The aim of this thesis is to develop a robust predictive method to identify optimal deep brain stimulation areas while minimizing the risks of side effects for patients. We have designed a workflow that integrates the parcellation of deep brain structures into consistent sub-regions. These sub-regions are then used to predict the effect of deep brain stimulation at a given point, using machine learning algorithms. By analyzing the model's predictions based on the position of the electrodes, an "optimal" region has been identified. This area is distinguished by significant clinical improvement while limiting the risk of side effects, both at the individual patient level and in relation to a brain atlas.This predictive method paves the way for personalized deep brain stimulation treatments, optimizing therapeutic efficacy while reducing risks for patients.
