神経内科学教室竹宮敏子教授退任記念特別号 Peripheral hemodynamics was assessed in patients with spinocerebellar degeneration (SCD), Parkinson disease (PD) and cerebrovascular disease (CVD), based on the measurements of various plethysmographic parameters. The wave height Hv (mV/V) in digital plethysmography (DPG), the wave height P (mV/Vsec) in velocitied plethysmography (VPG), and the value of |b/a| and |d/a| in accelerated plethysmography (APG) were compared between the patients in each group and their age- and sex-matched normal controls. Furthermore, the changes from the baseline in the values of Hv and the dicrotic index (DI) were examined in patients who presented with orthostatic hypotension. The value of Hv, among the parameters measured in DPG, is considered to roughly reflect the status of the peripheral circulation, since they are determined by organic factors, secondary modulators and other relevant factors. In this study, the peripheral circulatory condition was revealed to be poor in patients of SCD, PD and CVD, based on the values in patients with the three diseases as compared with those in age- and sex-matched normal controls. Lower P values may be reflective of a decreaed blood volume. Furthermore, lower values of |b/a| and |d/a| suggest organic hardening and contraction-modulating disturbances, respectively, of the walls of arterioles.
