Original abstract

Problem: Morbus Parkinson stellt als eine der häufigsten neurologischen Erkrankungen eine große Herausforderung für das Gesundheitswesen dar und nimmt durch die steigende Lebenserwartung in Zukunft immer mehr an Bedeutung zu. Die Erkrankung ist durch kognitive und motorische Beeinträchtigungen gekennzeichnet, wobei das Ausführen von Doppelaufgaben, also zwei Aufgaben gleichzeitig, besonders beeinträchtigt ist. Solche Defizite beeinträchtigen das berufliche und soziale Leben enorm. Ziel dieser Arbeit ist es, sich auf die physiotherapeutische Anwendung von Dual Task zu konzentrieren, um zu untersuchen welche Methode die besten Auswirkungen erzielt. Methode: Für die Beantwortung der Fragen wurde eine selektive, sowie eine systematische Literaturrecherche durchgeführt. Es wurden vor allem die Datenbanken PubMed, GoogleScholar, SpringerLink und ScienceDirect, sowie der Bibliothekskatalog der FH JOANNEUM genutzt. Es wurde mit zuvor festgelegten Keywords recherchiert.Ergebnisse: Aus der Recherche ging hervor, dass das Hinzufügen von Dual Task-Aufgaben den Gang einschränkt wodurch sich das Sturzrisiko erhöht. Weitere Studien beobachten negative Auswirkungen auf den Gang und die Kognition. Allerdings lässt sich durch das Training von Dual Task auch eine Verbesserung der kognitiven Leistung feststellen. Es ist eine wesentliche Stärke von Dual Task Training, dass es ermöglicht, das Gehen mit kognitive Aufgaben zu trainieren, was sich in großem Umfang auf reale Lebenssituationen übertragen lässt. Diskussion: Studien zur Anwendung von Dual Task zeigen gemischte Ergebnisse. Während einige, eine Verbesserung der Leistung feststellen beobachten andere negative Auswirkungen auf den Gang und die Kognition. Das könnte darauf zurückzuführen sein, dass jede Untersuchung verschiedene Dual Task-Methoden verwendet. Zusätzlich könnten die individuellen Eigenschaften der Patient*innen und der Umgebung zu den unterschiedlichen Ergebnissen beitragen. Dennoch behaupten einige Studien, dass Dual Task eine wirksame Behandlungsstrategie in der Rehabilitation ist. In Zukunft werden weitere Studien benötigt welche sich vor allem auf einheitliche Dual Task-Methoden fokussieren. Problem: Parkinson’s Disease counts as one of the most common neurological diseases and represents a major challenge for the healthcare system. It will become increasingly important in the future due to rising life expectancy. The disease is characterized by cognitive and motor impairments, whereby the performance of dual tasks, i.e. two tasks at the same time, is particularly impaired. Such deficits have an enormous impact on work and social life. The aim of this thesis is to focus on the physiotherapeutic application of dual task in order to investigate which method achieves the best effects. Method: A selective literature research as well as a systematic literature research was carried out to answer the questions. The databases PubMed, GoogleScholar, SpringerLink and ScienceDirect, as well as the library catalog of the FH JOANNEUM were used. The research was carried out using previously defined keywords.Results: The research showed that the addition of dual-task tasks restricts gait, which increases the risk of falling. Other studies have observed negative effects on gait and cognition. However, dual task training has also been shown to improve cognitive performance. Furthermore, it is a major strength of dual task training that it enables walking to be trained with cognitive tasks, which can be widely transferred to real life situations.Discussion: Studies on the use of dual task show mixed results. While some find an improvement in performance, others observe negative effects on gait and cognition. This could be due to the fact that each study uses different DT methods. In addition, the individual characteristics of the patients and the environment could contribute to the different results. Nevertheless, some studies claim that dual task is an effective treatment strategy in rehabilitation. In the future, further studies are needed that focus primarily on standardized DT methods.
