Original abstract

Einleitung: Der Morbus Parkinson kann im beruflichen Alltag häufig zu Herausforderungen führen. Zudem nimmt die Erwerbsfähigkeit mit Erkrankungsbeginn rapide ab (Murphy et al., 2013). Es gilt für die Ergotherapie, die Partizipation, sowie das Wohlbefinden am Arbeitsplatz zu fördern und die subjektive Leistungsfähigkeit von Betroffenen zu steigern (WFOT, 2016). Ziel: Das Ziel dieser Arbeit ist es, herauszufinden, wie sich der Morbus Parkinson auf die Arbeitsfähigkeit aus ergotherapeutischer Sicht, im Einzelfall auswirkt. Die Erkenntnisse dieser Arbeit sollen als Grundlage für Ergotherapeut*innen dienen, welche berufstätige, an Parkinson erkrankte Personen behandeln, um Partizipation und Wohlbefinden am Arbeitsplatz zu fördern. Methode: Zur Beantwortung der Forschungsfrage wurde eine qualitative Einzelfallstudie durchgeführt, in welcher eine, an Parkinson erkrankte Person, am Arbeitsplatz beobachtet wurde. Es wurde mit Hilfe von Betätigungsanalysen erhoben, bei welchen Performanzfähigkeiten, Schwierigkeiten in der Ausführung von drei beruflichen Tätigkeiten auftreten und wie dabei Umwelt- und Kontextfaktoren diese Tätigkeit beeinflussen. Die Ausführung der Performanzfähigkeiten wurde mittels drei Schwierigkeitsgraden beurteilt. Die Betätigungsanalysen wurden quantitativ als auch qualitativ nach Kuckartz (2018) ausgewertet. Ergebnisse: Die Auswertung der Betätigungsanalysen zeigte, dass sich die Erkrankung insbesondere in der Ausführung der motorischen Performanzfähigkeiten auswirkt. Diese Auswirkungen erschweren den beruflichen Alltag, in diesem Fall, jedoch nur minimal. Zudem wurden Performanzfähigkeiten vorwiegend dem Gesundheitszustand entsprechend adäquat ausgeführt. Unterstützende, sowie hemmende Umwelt- und Kontextfaktoren, beispielsweise das soziale Umfeld, konnten im Rahmen der Betätigungsanalysen erhoben werden und haben einen erwähnenswerten Einfluss auf die Arbeitsfähigkeit. Conclusio: Die gezielte Förderung von Performanzfähigkeiten, ob beeinträchtigt oder nicht, stellt einen Bestandteil der ergotherapeutischen Behandlung einer berufstätigen, an Parkinson erkrankten Person, dar. Hierbei sollten Umwelt- und Kontextfaktoren unbedingt berücksichtigt werden. Dies kann in Form eines Job Coachings, im Rahmen der Arbeitstherapie, erfolgen. In weiterführender Forschung wäre es interessant zu erheben, ob ein frühzeitiger Arbeitsplatzverlust, durch zunehmende Ergotherapie in der Arbeitstherapie, verhindert werden könnte. Background: Parkinson's disease (PD) patients often face challenges in the workplace due to the decrease in work ability with the onset of the disease (Murphy et al., 2013). Occupational therapy (OT) aims at enhancing the participation, well-being and subjective performance at work in affected patients (WFOT, 2016). Aim: The aim of this work is to find out how PD affects the ability to work in an individual case from an OT perspective. The findings help OTs treating employed persons with PD to enhance participation and well-being at work. Methods: In a qualitative single-case study a PD patient was observed at the workplace engaging three occupations. Challenges in performance skills and the influence of environments and contexts were assessed by occupation-based activity analyses. The challenges were graded in three levels of difficulty. The evaluation of the occupation-based activity analyses was carried out quantitatively and qualitatively according to Kuckartz (2018). Results: The evaluation of the occupation-based activity analyses showed that PD effects particularly the motor performance skills. In this case, these effects have only a minimal impact in the daily occupational routine. Most of the performance skills were adequately performed according to the state of health. Moreover, supportive and inhibiting environments and contexts, e.g., social environment, were assessed which showed an influence on the ability to work. Conclusion: An important component of OT treatment of an employed PD patient is the advancement of performance skills, irregardless of whether they are affected or not. Furthermore, environments and contexts should be considered. This could be a part of job coaching, as an element in work-related practice. Further research is needed to find out, whether increased OT in work-related practice could prevent an early loss of job due to a PD.
